Exemption Flyer

To request exemptions, students need fill out this form linked below. It was also emailed to students.

THE EXEMPTION FORM CAN ONLY BE COMPLETED ONE TIME.

• You must maintain an 80 or above semester average in the class for which the you are seeking an exam exemption.
•  
• You must meet disciplinary expectations to qualify for exam exemptions.

You will lose your semester exemptions if you:

• Receive a disciplinary assignment to on-campus suspension (AC/ISS)
• Receive a disciplinary assignment to off-campus suspension or alternate education program(s) [BIC and/or JJAEP]

Once you submit this form, you will receive a receipt of your submission to your MISD Google email account. The email receipt will come from a Google Forms email address - not in district. Please check your MISD Google account for the response receipt.

• These forms will go live on May 16th at 7:00 AM and will close on May 22nd at 11:59 PM.
• All students will be sent the exemption form via MISD email
• Students must be logged-in to their MISD email accounts to access the form, and they will receive an email receipt for their responses after they press submit. If a student does not receive an email receipt, they have not correctly submitted the form.
• If a student is experiencing trouble logging in to their MISD email, please send them to the library.
